The fee is:
$60 for work estimated to be $1000 or less
$70 for work estimated between $1000.01 and $2000
$78 for work estimated between $2000.01 and $3000
$84 for work estimated between $3000.01 and $4000
$84 plus $4 for each additional $1000 or fraction thereof for work estimated over $4000.
The current state levy is 2%.
